At YouFirstClinic, we strive to make your experience as a new patient as seamless and comfortable as possible. Follow our steps below for the best possible experience with Thrive.

Bring key documents
Make sure you have your insurance card, a government- issued photo ID, and payment method (cash, check, credit card).

Medical history
A list of your current medications and dosages (this includes over-the-counter medications, and all vitamins or supplements).

Already scheduled?
If you made your appointment, make sure to fill out your new patient paperwork through your patient portal.
4.
Complete paperwork
Complete your New Patient paperwork prior to your appointment. You may complete this online or come early to your appointment to allow enough time to fill out all necessary paperwork.
